Using a simple and intuitive app we assisted a client to engage directly with the patient using tools to monitor their medication adherence, review their health metrics with biometric measurements and clinical assessments and always have access to their care team using secured chat and video calling access. A personal care plan section showed them how they were progressing with managing their chronic disease and tracking personal health coaching goals in activity, nutrition and lifestyle
